- Every person: name, role, and whether they started before or after 14 August 2026
- Foundation training: date completed, certificate held, and where it is filed
- Whether they hold a relevant role, and therefore whether advanced training applies
- Advanced training: due date, date completed, certificate held
- Refresher due dates, calculated two years on
- A last verified field, so the register itself is evidence of monitoring, not just of intent
